Finding Your Care Passport Portal User ID

After you have created and confirmed your Care Passport Portal account, your employer can add you to a Home and a Role within that home.
In order to add you to a Home, your employer will need the following information from your Portal account:
User ID
First Name
Last Name
Date of Birth

To find your Care Passport Portal User ID, login to your portal account and go to the My Profile tab.
You can find your Care Passport Portal User ID at the top of your account information as shown below.
PLEASE NOTE: your User ID is a four-digit number like the one highlighted in the screenshot below. It is separate from the Username you have created for logging in to the Portal.
