The LTC Training Care Passport is a standardized training certification program that gives staff working in long-term care homes access to standardized orientation training and a way to verify completion of the training for their employers.

There are two stages to this process:

  1. Registering for and completing the training micro-credentials

  2. Creating your personal account in the Care Passport Portal

This guide covers registering for and completing the Care Passport training micro-credentials.

Links to instructions related to the Care Passport Portal are provided at the end of this guide under Creating and Managing Your Care Passport Portal Account.

Registering for Your Care Passport Micro-credentials

Care Passport training is assigned as a block of micro-credentials based on the LTC home you are working in and your staff role in that home. Be sure to confirm with your employer which training block you need to register for.

To register for your Care Passport training micro-credentials:

  1. Log in to mylambton.ca

  2. Open the LTC Orientation Training Registration Form. (If you are denied access to the form, it means you are not logged into mylambton).

  3. Complete and submit the form.

You will get access to your training micro-credentials in D2L within two business days after you have submitted the form.

Achieving a Pass Grade in Your Micro-credentials

Before starting your training micro-credentials, be sure to review the Achieving a Pass Grade in Your Micro-credentials instructions to give yourself the best chance of completing your training micro-credentials successfully.

Creating and Managing Your Care Passport Portal Account

Your Care Passport portal account maintains a record of your completed Care Passport training micro-credentials, which you can access in the Manage My Training tab.

Keep your username, user ID and password in a safe place so that you can access your portal account whenever you need to. Your employer will request that you share your course records as proof of completion of the training you are required to have as an orientation into the sector.

You and your employer can track completion of your Care Passport training micro-credentials in the Care Passport Portal.

Follow the How To Create Your Care Passport Portal Account instructions to create your portal account.
