Once it has been determined that you are eligible for BHER funding to cover the cost of your Care Passport training micro-credentials, there are a few steps you will need to do to get access to and complete your training micro-credentials.

You will receive an email at each step of the process, so it is important that you check your junk folder regularly in case your email provider redirects any of the emails there.

The steps of the process are also provided below for your reference.

Application and Registration Process

Be sure to follow each step carefully.

Missing a step could result in your work placement being delayed.

Step 1: Create a Care Passport Portal Account

Step 2: Submit the BHER Intake Survey

Step 3: Update Your Care Passport Portal Account

Step 4: Register for Your LTC Training Care Passport Micro-credentials

Step 5: Complete and Track Your LTC Training Care Passport Micro-credentials

Step 6: Submit the BHER Completion Survey

Step 1: Create a Care Passport Portal Account

Now that it has been determined that you are eligible for BHER funding, you will need to create a Care Passport portal account.

You can see the instructions for creating an account here: How To Create Your Care Passport Portal Account.

You can access the Care Passport Portal here: Care Passport Portal Login Page

Be sure to check the box to create a new Lambton College student account. This is required for you to get access to your LTC Training Care Passport micro-credentials.

Once you have created your Care Passport portal account and checked the box to create a Lambton College student account, your login credentials will be emailed to you within two business days. Be sure to check your junk mail folder

Step 2: Submit the BHER Intake Survey

To be enrolled in the BHER funding, you will need to complete a short 5-minute survey, which will be emailed to you.

This survey must be completed prior to you beginning your LTC Training Care Passport micro-credentials.

As a thank-you for completing the survey, you will receive a $10 gift card for Starbucks, Amazon, Indigo, Cineplex, Tim Hortons, or iTunes.

Step 3: Update Your Care Passport Portal Account

Once we receive confirmation that your BHER Intake Survey response has been received, you will be sent an email instructing you to do the the following three things:

Confirm Portal Request To Be Added To BHER

You will receive a confirmation email adding you to the BHER organization in the Care Passport portal. You need to select the Confirm button in this email to proceed.

Be sure to check your junk mail folder in case your email provider has filtered the confirmation email into junk mail.

Add Lambton College Student Number to Portal Account

Follow these instructions to add your Lambton College student number (also called a c-number) to your Care Passport portal account: Adding Your Lambton College Student Number to Your Care Passport Portal Account

Your placement may be delayed if student number is not added to your portal account, because your placement site will not be able to confirm that you have completed your LTC Training Care Passport micro-credentials.

Download Training Requirements

Once you have selected the Confirm button in the confirmation email, you can download a list of the LTC Training Care Passport micro-credentials you are required to complete before starting your placement.

Follow these instructions to download your list of required micro-credentials: Using the Manage My Training Tab to Filter and Download Your Training Requirements

Step 4: Register for Your LTC Training Care Passport Micro-credentials

Please Note: You do not need to register for all of your micro-credentials at once.

After you register for a micro-credential, you will have access to it in Lambton College D2L within two hours.

You will have access to each micro-credential for 14 days from the day your register in it. It is recommended that you register for your micro-credentials a few at a time, so that you can be sure you are able to finish each of them before the 14 day deadline.

In Step 3, you downloaded a list of the micro-credentials you are required to complete. Now, in Step 4, you’re going to register in your micro-credentials. We’re going to break that down into sub-steps to make things easier.

In the Manage My Training tab, beside each course code, there is a link symbol (it looks like this image-20240628-201224.png ) that you can click to be taken to the registration page for that micro-credential.

Step 4.2: Navigate to a Registration Page

Refer to the list you downloaded in Step 3. For each of the micro-credentials you are required to take, click on link for it in the Manage My Training tab to be taken to the registration page.

Step 4.3: Add a Micro-credential to Your Shopping Cart

On the registration page, click on the yellow Add to Cart button (it looks like this image-20240319-203017.png). Then come back to this page and select the next micro-credential you need to register in and again add it to your cart.

Step 4.4: Checking Out

Once you’ve added to your cart all the micro-credentials you want to register in now (remember, you don’t have to register in them all at once - you can do them a few at a time), click the Create an account or Login link at the bottom of the page (it looks like this image-20240319-203119.png).

Step 4.5: Logging In to Your Account

On the Create an Account or Login page, scroll down to the Online Returning Student section, click the Log in to Your Account button (it looks like this image-20240319-203325.png) and login using your Lambton College credentials.

Step 4.6: Finalizing Your Registration

Double check that your name, address, and contact information is correct on the three Student Information pages, and then click the Register button (it looks like this image-20240319-204021.png). Within two hours you will have access to your micro-credentials on Lambton College D2L.

Step 5: Complete and Track Your LTC Training Care Passport Micro-credentials

Accessing Your Micro-Credentials

To access your micro-credentials, go to www.mylambton.ca and login using your Lambton College credentials.

Select the D2L tab in the menu bar to be taken to D2L. On the D2L homepage, under My Courses, you will see all of the micro-credentials you are currently registered in.

Before you start any of your micro-credentials, review this short set of instructions outlining how to give yourself the best chance of achieving a Pass grade in each of your required micro-credentials: Achieving a Pass Grade in Your Micro-credentials

Tracking Your Completed Micro-credentials in D2L

Checking That You’ve Received a Pass Grade for Each Micro-credential

As shown in the Achieving a Pass Grade in Your Micro-credentials instructions, it is important that you check the Grades tab in each micro-credential to determine if you have received a Pass grade or not.

Tracking Compliance in the Care Passport Portal

Each time you successfully complete a Care Passport micro-credential, it will show as Validated on the next business day in the Manage My Training tab of your Care Passport portal account.

The supervisor at your placement site will also be able to view a report showing which micro-credentials you have completed.

Step 6: Submit the BHER Completion Survey

At the end of your placement, you will receive an email with a link to a short 5-minute survey similar to the BHER Intake Survey you completed in Step 3.

As a thank-you for completing the survey, you will receive another $10 gift card for Starbucks, Amazon, Indigo, Cineplex, Tim Hortons, or iTunes.
