Instructions

  1. Go to the Care Passport Portal homepage Sign in page at carepassport.ca

  2. Select the Sign Up SignUp button

  3. Create your account by filling in your information on the registration page. Be sure to record the username and password you have created for the Care Passport Portal. You will need it each time you login to the portal.

  4. Employee creates portal account and checks the box to create a new Lambton College student number.

    1. NOTE: if you have an existing Lambton College student number, checking the box will tell the Lambton College system to update your contact information using the information you have entered in the Care Passport portal.

  5. After you select Submit on the registration page, you will receive a confirmation email from CarePassport@lambtoncollege.ca (Be sure to check your junk mail folder in case the email has been sent there by your spam filter).

    Fill in all of the required information and remember the following:

    1. If you’ve been a student or employee at Lambton College:

      1. Make sure you use the same name as you did when you attended or worked at Lambton College

      2. Be sure to insert your student number (c01234567)

    2. If you’ve never studied or worked at Lambton College:

      1. Select “Request a new student number”

    3. Consent to the terms and conditions.

  6. Confirm Registration - You will receive an email from carepassport@lambtoncollege.ca (check your junk mail) and you will have to confirm the email to activate your portal account. This email will confirm:

    1. Your username, and

    2. Your 4-digit portal ID

  7. Once you have confirmed your registration, you can login to your Care Passport Portal account at carepassport.ca/login

  8. When you login, you will be taken to the My Profile tab of your Care Passport Portal Dashboard. Use the My Profile tab if you need to update any of your account information.

    1. Please Note: When you login to your portal account for the first time, make a note your User ID (as shown in the image below). You will need to give your portal User ID to your employer when they add you as an employee in the portal. See Finding Your Care Passport Portal User ID for more information.

  9. Select the Manage My Training tab to see the status of your Care Passport training micro-credentials. If you are employed at more than one Home or assigned to more than one Role, you can use the filters shown below to display your status for a specific Home and/or Role. If you do not select any filters, all of your micro-credentials will be displayed.

  10. You can scroll through the list of your micro-credentials and view the status for each one. If the Status column is empty, it means you have not successfully completed that micro-credential.

  11. For any micro-credential that has been successfully completed the Status column will say Validated. If the Valid For column is empty, that micro-credential does not have an expiry date. For micro-credentials that do have an expiry date, the Valid For column will indicate how many years they are valid for. If you have completed a micro-credential that has an expiry date, the Expiry Date column will tell you the date after which it will no longer be valid.

  1. you will receive a second email that welcomes you to the portal. This welcome email will include:

    1. First name;

    2. Last name;

    3. Your username;

    4. Portal ID; and

    5. Links to how-to articles

Please Note: Once you have completed these 5 steps, you will need to provide your employer with the following details so that they can assign you to a home and a role - this must happen before you can begin registering for courses:

  1. First Name

  2. Last Name

  3. Birth Date - written out Month DD, YYYY

  4. Portal ID (portal ID can be found under the Profile tab)
