If you have forgotten your password for the Care Passport portal, you will need your portal Username to request a password reset.

Instructions

Go to https://lcpt-prod.herokuapp.com/login and select Forgot Password.

Enter your portal Username and select Email me a temporary password.

Remember to check your junk folder if you have not received your temporary password within 20 minutes.

Once you have received the email with your temporary password, enter it in the Enter your temporary password field.

Create and confirm your new password and select Update password.
